Our services may be covered by your insurance provider if you have out-of-network care included in your insurance plan. In general, we participate in out-of-network coverage with commercial insurance plans. As a courtesy, we will file claims to your insurance for you so that you may get reimbursement according to your plan’s benefits. We do not participate in Medicare or Medicaid and do not submit to these plans for reimbursement. If you do not have out-of-network benefits or prefer to pay outside of your insurance plan, cash pay rates are available on inquiry. Please allow up to 2 business days for insurance verification after submission of this form.

Appointments, including your initial evaluation in the office, are usually 55 minutes. Longer sessions are available upon request.

Please wear clothing you feel comfortable moving in. We have some clothing (shorts, t-shirts) in the office if you require a change of clothing.

Our office does not have a lobby. If the door is locked when you arrive, it means we are still with our previous client. At the scheduled time of your appointment, we will welcome you into the treatment area. There is a small bench outside the door for you to sit on if you arrive early.
If you require the restroom in the Union Square office, it is in the main hallway as you approach the office directly across from the second stairwell door. At your first appointment, we will discuss your care, do a full body assessment, and agree on a treatment plan. -
Home sessions are sometimes available upon request, subject to therapist availability. Home visits may be subject to a travel fee depending on the location and travel required by the therapist.

We do not perform massage, and we are not a licensed massage therapists. As a physical therapists we am highly trained in many modalities of manual therapy, which improve your body’s flexibility and mobility. Some types of manual therapy we provide include visceral manual therapy, neurological manual therapy, myofascial release, functional manual therapy, joint mobilizations, craniosacral therapy, lymphatic drainage, and counterstain therapy. We do believe massage can be a useful adjunct to physical therapy depending on your diagnosis, and we are happy to refer you to a massage therapist we work with if you would like to supplement your treatments with massage.
